ANALOG JIU JITSU LLC

WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK (State of Texas)

1. Acknowledgment of Risk

I understand that participation in Brazilian Jiu Jitsu and related martial arts activities at Analog Jiu Jitsu LLC (“Analog Jiu Jitsu”) involves inherent risks.

These activities include, but are not limited to:

• Physical contact with instructors and other students
• Grappling, takedowns, joint locks, and positional training
• Use of mats, equipment, and training space
• Participation in drills, live training, and sparring

I understand that these activities may result in serious injury, including but not limited to sprains, fractures, concussions, joint injuries, paralysis, or death.

I voluntarily assume all risks associated with participation.

2. Assumption of Responsibility

I understand that:

• I am responsible for monitoring my own physical condition.
• I will immediately stop participation if I feel pain, dizziness, or unsafe conditions.
• I will inform an instructor if I am injured or feel unwell.

I acknowledge that I am voluntarily participating and assume full responsibility for my safety.

3. Release of Liability

In consideration for being permitted to participate in activities at Analog Jiu Jitsu, I hereby release, waive, discharge, and covenant not to sue Analog Jiu Jitsu LLC, its owners, members, managers, employees, contractors, agents, and affiliates (collectively, the “Released Parties”) from any and all claims, liabilities, damages, demands, or causes of action arising out of or related to my participation, including claims arising from the negligence of the Released Parties, to the fullest extent permitted by the laws of the State of Texas.

This release applies to personal injury, property damage, or wrongful death.

4. Indemnification

I agree to indemnify and hold harmless the Released Parties from any claims, costs, or expenses (including reasonable attorney’s fees) arising from my participation in activities at Analog Jiu Jitsu.

5. Medical Consent

In the event of an emergency, I authorize Analog Jiu Jitsu to obtain medical treatment for me. I understand that I am responsible for any medical costs incurred.

6. Severability

If any portion of this agreement is found unenforceable under Texas law, the remaining provisions shall remain in full force and effect.

7. Acknowledgment

I have read this agreement carefully and agree to all terms above.

I am the parent or legal guardian of the minor participant named above. I have read and understand this agreement. I consent to the minor’s participation and agree to the terms of this waiver on behalf of the minor and myself.

MEDIA & PHOTO RELEASE

Analog Jiu Jitsu may photograph or record classes, events, or training sessions for marketing and promotional purposes, including use on social media, website, and printed materials.
